I put this on my '71 WAY back when in the '70's to get a firmer brake pedal (along with the 19mm MC) in place of the proportioning valve on the bulkhead. Fits right in. Made by Tilton I'm assuming by the handle. I'm sure someone made the base to fit the existing holes in the bulkhead. I doubt I'd ever find the original receipt!!

Any idea who made this and modified the Tilton part? ( I don't need to get into the debate on the value of the OEM part! Been there, done that)

I see a similiar part without the base on the Tilton site but this fits right in.
