Three "things" had to happen...

Someone disabled the mechanical locks. This is the MAIN safety failure.
You lose the cylinders AND the cables with the locks functional, and the car stays UP.
It is a double headed nightmare to get down if left 6 feet high, but the car doesn't fall...

Disabling the safeties on any lift is negligence in my book.

The cables failed. The cables have a service life and should be replaced at least every ten years.
My BendPak lift service manual states 5. Mine are 9 years old now...

Light cars and low "cycle" life extends cable life IMO. I DO inspect and lubricate my cables ever year. They take special lube specifically for cabling.

Every side post lift I've worked on has a hydraulic ram on both sides. It might be possible to have a single ram lift, but I've never seen one.

The ram failed on the left side.

So, even with triple redundancy that car dropped.

Looked like a nice car!

I have a rule in my shop.
A car left chucked up on the lift is ON the first safety lock overnight. If the lift shits the bed overnight the car is only 18" high and can be removed with a floor jack and jack stands.

You can get the car off of the lift BEFORE you have to call the customer...

I wonder if the car was restored? Probably. Was any of the glass still okay? If it had been my car, I would have pissed on it prior to having filed an insurance claim. I guess George would have been kind enough to have his insurance company cover it. Yes? Definitely off topic now, but tangents can be fun.
